What is Melasma, and How Does It Differ from Blemishes?

Melasma is not simply a problem of excessive melanin production causing pigmentation on the skin.

It involves pigment cells called melanocytes becoming excessively activated in the deeper layers of the skin,

and the melanin produced by these cells spreading widely and repeatedly,

infiltrating even into the dermis, resulting in a complex long-term pigmentation disorder.

Especially external stimuli like UV light or hormonal changes stimulate melanocytes,

and during this process, microscopic inflammatory reactions and vascular factors within the skin work together.

It is characterized by pigment spreading in a blurry, wide pattern rather than a simple spot.

Histologically, the number of melanocytes is within the normal range,

but the size of the cells and their dendritic processes (dendrites) significantly increase,

causing melanin to be delivered in greater amounts and to a wider area to surrounding cells.

Additionally, in some patients, subtle capillary dilation or flushing reactions are observed in the melasma area,

and in such cases, it is sometimes described as a separate category called vascular melasma.

Melasma is less a disease in itself and more a result of skin reacting easily

and forming due to various stimuli in a structurally unstable state.

Therefore, treatment that addresses its root causes and structure, rather than simple pigment removal, is crucial.

While dual toning is also needed for general pigmentary disorders, it is especially essential for melasma, where conventional toning alone is insufficient.
